In 2009, Vermont received a 6-year grant from SAMHSA (Substance Abuse and Mental Health Services Administration) to improve the system of care for transition-aged youth and young adults 16 through 21 at risk of being out-of-school and/or incarcerated. Each service area was asked to create a plan to address the most compelling issues facing the target population in the region.
